Please provide in these as two separate figures: - one for severe and one for death - and a figure of how many deaths there were in total (as an incident can refer to more than one death) | |
| Answer To Question 1: The enclosed data shows all major and catastrophic patient safety incidents for the dates requested. It is not possible to give a specific number of deaths that this relates to due to this being a manual process of looking through each case individually this would take more than 18 hours to complete.
