Question Number 1:
Can I request the number of patient safety incidents that resulted in a severe or death degree of harm to the patients (as defined by ¿´Æ¬Èí¼þ England here) and what month and year they took place from 2010 to present please.

Please provide in these as two separate figures:
- one for severe and one for death
- and a figure of how many deaths there were in total (as an incident can refer to more than one death)
 
Answer To Question 1:
The enclosed data shows all major and catastrophic patient safety incidents for the dates requested.

It is not possible to give a specific number of deaths that this relates to due to this being a manual process of looking through each case individually this would take more than 18 hours to complete. Exemption notice applied.
