Question Number 1: How many fatalities have been due to cancer wrong diagnosis or failure/delay diagnosis in the last three years (Between 2021-up to and including any data held for 2024)? For example: The number of incidents where the Primary Cause is ‘Wrong Diagnosis’ or ‘Failure/Delay Diagnosis’ and where both injury codes ‘Cancer’ and ‘Fatality’ are recorded? Can this be broken down by year and presented in PDF format? | |
Answer To Question 1: The Trust are unable to answer this request as this is not information that the Risk Management department would hold, we would be able to look into how many incidents there are of delayed or missed diagnosis and if those patients had since passed away, but we would be unable to determine if their death was attributable to a late/missed cancer diagnosis. | |
