Answer To Question 1: Q1 All staff have to complete mandatory online training on Equality, Diversity and Inclusion (EDI) (refreshed every 3 years). Answer To Question 2: The Trust have 2 main protocols for reporting incidents – 1. Where incidents relate to patient safety there is the PSIRF protocol: http://intranet/quality/patient-safety-incident-response-framework-psirf/ 2. And for all incidents, including regarding equality rights, disabilities and carers, we have our standard DATIX system: http://intranet/media/3orjqvtu/datix_how-to-report-an-incident_lfpsev2.pdf The Trust also have our patient experience team to take complaints (sft.pals@nhs.net) and staff can raise their issues with the Freedom to Speak Up Guardian if they feel their issues are not handled well. | |
