How do I access my patient records?
You have the right to find out what information we hold about you as a member of staff or as a patient. This is called a right of access or Subject Access Request (SAR). You exercise this right by asking us for a copy of the information Salisbury Foundation Trust Hospital holds about you.
You may ask to view your patient/health records in person, or you may ask for a copy of your health records.
In most circumstances, the information will be provided to you free of charge. Salisbury Foundation Trust Hospital may raise a charge if there is an excessive amount of information in your record. In these circumstances we will inform you of the potential cost before responding to your request so that you can make a decision on whether you would like to proceed.
We are required to supply this information to you within 30 calendar days from the date the Trust received the request. Should additional time be required because of the complexity of the request we will write to let you know.
Please note, we will need to confirm your identity before disclosing information or releasing photocopies.
If you would like to receive a copy of your medical records, report a concern or inaccuracy within your record or would like to restrict who your medical data is shared with, please speak to your clinician or contact any of the people listed below. They will be happy to help.
